Job Description The Clinical Social Worker will serve as an integral part of our onsite primary care team, responsible for helping patients improve their physical and mental health. The Clinical Social Worker functions as a core member of our enhanced Primary Care team that involves the patient's primary care provider, a psychiatric consultant, and other mental health and social service providers in the clinic and community. The Clinical Social Worker is responsible for coordinating and supporting mental health and social care within the clinic and for coordinating referrals to clinically indicated mental health and social services outside the clinic. The Clinical Social Worker will integrate clinical expertise with community resources to address the needs of our community members in their homes. Responsible for conducting intake and all required paperwork for program enrollment. Responsible for completing comprehensive assessments in the areas of mental health, nutrition, psychosocial needs, substance use disorders, education/vocation, goals, parenting skills, and home safety for each client. Provide brief interventions using evidence-based techniques such as Behavioral Activation, Problem-Solving Treatment, Motivational Interviewing, or other treatments appropriate for primary care settings. Monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications. Keeps accurate, up-to-date records on clients served including contact summaries, progress notes, support service plans, and program specific reports. Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments (e.g., CBT, IPT) as clinically indicated. Support psychotropic medication management prescribed by PCPs, focusing on treatment adherence, side effects and other complications, and effectiveness of treatment. Facilitate treatment plan changes for patients who are not improving as expected in consultation with the PCP and the psychiatric consultant. Participate in regularly scheduled (usually weekly) caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patient's PCP. Determines the nature of the client's situation by interviewing clients, assessing medical, psychological, emotional and social information and making on-site home visits. Document patient progress and treatment recommendations in the EHR system so they can be easily shared with PCPs, the psychiatric consultant, and other treating providers. Facilitate referrals for clinically indicated services outside the primary care clinic (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment). Serves as liaison with other community agencies, schools, behavioral health, and department initiatives. Obtains assistance for the client by referring him/her to community resources, arranging for appointments and establishing rapport with other agencies. Advocates for clients when there is a problem in the service delivery system. Maintains operations by following policies and procedures, participating in quality reviews and reporting needed changes. Maintains client confidence and protects operations by keeping information confidential. Perform other job-related duties as assigned. This role is based in Lost Hills, CA with occasional travel to other wellness centers in the Central Valley. Qualifications Current and valid LCSW license issued by the State of California Board of Behavioral Sciences. 2+ years of clinical experience in a mental health setting, including direct service delivery to clients. Experience in case management and coordination of community resources is highly desirable. Experience with screening, assessment, and treatment planning for common mental health and/or substance use disorders, when appropriate. Must speak Spanish fluently. Valid California driver's license and insurance. Additional Information Pay Range: $90,000 - $100,000 annually and is eligible for a discretionary bonus. Final compensation will be dependent upon skills & experience. Additionally, this position is eligible for a monthly car allowance.
