How You'll Make a Difference:

Arkos Health is seeking a Community Health Worker. The Community Health Worker will be responsible for providing ongoing support and expertise throughout assessment and planning of individual members' needs. This role is patient facing and similar to a community health worker. The overall goal of the position is to enhance the quality of patient management and to promote continuity of care and cost effectiveness through therapeutic interaction, supportive education, active listening, advocacy, collaboration with providers and community referral.

Essential Duties and Responsibilities:
  • Identify resources (internal & external) resources for health and community data
  • Demonstrate ability to analyze data and information critically to integrate findings into clinical operations, processes, and programs
  • Provide services such as: community referrals, supportive services, therapeutic interaction, crisis intervention, member advocacy in support of member and overall goal
  • Maintain deep understanding of the Social Determinants of Health and how they influence health status in an effort, intervening as appropriate to help not only prevent decline but improve outcomes
  • Assist patients with managing their basic health needs
  • Conduct member outreach through home visits and welfare checks to assess member for program participation and establish lines of communication for ongoing engagement
  • Provide active and passive, culturally sensitive education on key health topics and concerns to member-clients
  • Assist patients with navigating the healthcare system to increase engagement and compliance, which would include assistance with appointment scheduling
  • Creation of and participation in community events to increase awareness of our services
  • Present formally and informally, key health education and information to the community, individuals and other professionals
  • Perform other duties as assigned to support the overall effectiveness of the organization

Qualifications and Skills:
  • High school diploma/GED
  • Must have current BLS/CPR certification
  • Must have proof of current TB test
  • Current Hep B test- or waiver
  • Current Flu shot- or waiver and mask
  • Experience interacting with people with behavioral health issues
  • Must have valid driver license and appropriate liability insurance
  • Must be comfortable driving in the field up to 80% of the time and reporting to office when not actively scheduled in the field
  • Community Health Worker Certification or previous experience preferred.
  • Computer Skills: Proficient with Microsoft Office applications (Word, Excel, Outlook, and PowerPoint)
