Job Description
Optimus Health Care is a Federally Qualified Health Center with sites operating in Bridgeport, Stamford and Stratford, CT. We are the largest provider of primary health care services in Fairfield County, proudly serving all patients regardless of their ability to pay. We are currently recruiting for a full-time Bilingual Spanish LIcensed Clinical Social Worker or Bilingual Spanish Licensed Master of Social Worker to join our collaborative team in Bridgeport, CT POSITION SUMMARY Provides direct services to Optimus' patients and families with the goal of developing patient independence and self-sufficiency, when applicable. Also provides treatment for patients who are diagnosed with a mental illness or refer to appropriate service provider. Oversees, administers, coordinates and/or assists in other designated Optimus programs and functions as assigned. ESSENTIAL FUNCTIONS & RESPONSIBILITIES 1. Provides clinical behavioral health services and some case management, information & referral services to individuals and families. This includes psychosocial assessments and psychotherapy, treatment planning, collaboration with and referral to other members of the health care team internal and external. 2. Maintains appropriate documentation. 3. Generates initial treatment plans and updates every three (3) months. Discharge summaries are generated thirty (30) days after patient transferred, discharged or patient terminated treatment. 4. Responsible for department operation during Director's absence. 5. May provide supervision, consultation and oversight for some of the designated OHC programs and staff. This includes the ongoing and changing grant-funded programs at OHC and the clinical and non-clinical staff. These responsibilities also include conducting performance evaluations, in-service training, task assignments, and supervision of staff which include goal planning and career development. 6. Representation of the Social Services Department and / or OHC as a liaison at agency and community meetings. May delegate some of this responsibility to other staff. 7. Assist in program development as directed by the Director of Behavioral Health and / or Administration. 8. Responsible for the accurate and complete preparation of encounter information including appropriate coding for diagnosis and procedure; DSM-5 and CPT Coding to ensure prompt billing for services rendered. PROGRAM COORDINATION 1. Coordinate and implement assigned program as required per funding source 2. Collect monthly data from all participants to ensure participation in designate program, activity level and patient needs are met. 3. Conduct regular assessment of curriculum to ensure program is functioning as implemented. 4. Provide refresher (booster) training as need on how to apply the program to be able to extract required reporting data. 5. Prepare and submit report to funding source as required (monthly and/or annually) 6. Present at annual conferences as needed. ADDITIONAL GENERAL REQUIREMENTS Professional positive attitude, vision, understanding of customer service principals, trustworthiness, and excellent interpersonal skills to successfully accomplish tasks necessary to meet high standards of ethical and social responsibility required by this position. JOB QUALIFICATIONS/REQUIREMENTS EDUCATION: Masters' Degree in Social Work from an accredited university and EXPERIENCE: A minimum of two (2) years of related experience. Some behavioral health experience preferred in the areas of culturally diverse populations. Combination of clinical, crisis intervention, systems, case management, conflict-resolution, behavior modification, program development, child development, state and local resources, and group skills needed. Good verbal and written communication skills are required. Basic computer skills desired Familiarity with tele-medicine and prior experience a plus LANGUAGE SKILLS: Working Knowledge of Spanish Required. LICENSURE / CERTIFICATION: Licensed under the State of Connecticut Department of Public Health. A valid driver's license is also required.
