The Bedford-Stuyvesant Family Health Center (BSFHC) is a 501C3 Federally Qualified Health Center (FQHC) that serves all of the primary health care needs of families in the heart of North and Central Brooklyn for over 40 years. Our mission is to provide the most professional, courteous and highest quality health care, with dignity, to those we serve, especially the undeserved population, without regard for ability to pay.

The health center's Communities of Color (CoC) initiative aims to increase awareness in the community about health-related issues to improve the quality of care and delivery of services targeted towards women of color. The program will achieve this goal by linking community members to program services. The Outreach Health Educator will be providing health education to clients on available program services, health maintenance, and how barriers of adherence can be addressed to improve client's quality of care in collaboration with the client's care team.

Bilingual Spanish.

Required field about 75% of the time and take mass transit to travel throughout New York City.

Grant-funded: Contingent upon continuation of grant funding from year to year.

Duties and Responsibilities include but not limited to:
  • Develop partnerships at the community-level to identify women who are underserved, uninsured, out-of-care, especially people living with HIV (PLWH) as well as those in need of access to behavioral health and chronic disease management.
  • Conduct outreach activities at various community organizations and healthcare settings to raise awareness on insurance coverage and health-related issues providing information on services offered by BSFHC through supporting programs.
  • Educate clients on program services and patient medication management strategies.
  • Engage other community-based organizations, both HIV-specific and non-HIV-specific, to develop and maintain an outreach database.
  • Perform outreach to clients who have lost contact or require follow up on medical appointments through various communication methods (i.e., phone calls, letters, email).
  • Conduct outreach presentations at events such as conferences and healthcare settings throughout the Brooklyn region.
  • Document interventions and follow up client engagement in appropriate system (s) or administrative tracking tools for clients for data collection purposes and submission for accurate reports.
  • Utilize social marketing tools and collaborate with partners to identify and contact hard-to-reach populations who are underserved and uninsured.
  • Support the PrEP, HIV & HEP C programs by referring partners and community to accessing prevention services, particularly among MSM of color. .
  • This position requires the candidate to be on the field about 75% of the time and take mass transit to travel throughout Brooklyn.
  • Perform additional duties as requested by the Supervisor.

Knowledge, Skills & Abilities:
  • The successful candidate must have an understanding of the community's cultures, ethnicities, and knowledge of community assets and resources necessary to respond positively to its challenges. Knowledge of Brooklyn-based systems of care.
  • Ability to work independently and document all work performed.
  • Ability to communicate effectively and build positive relationships with a variety of people, especially underserved and hard-to-reach populations, and work in a team environment.
  • Ability to prioritize goals and objectives to deliver mission-critical services in a timely manner.
  • Demonstrate effective customer service providing culturally and linguistically appropriate care to all customers, clients, and patients.
  • Experience working with HIV/AIDS, LGBTQ, and communities of color.

Education Required
  • Bachelor's degree required.

Other Related Skills/Experience:
  • Maintain HIPPA patient/employee confidentiality
  • Knowledge of EMR systems a must
  • Excellent interpersonal skills must demonstrate professionalism, courteous and respectful attitude in dealing with patients, team members, families and significant others
