As a Population Health Coach you will interact with patients via telephone, with positive, motivational counseling sessions which are aimed at maintaining compliance to physician-prescribed medical protocols as well as facilitating informed patient choices related to a healthier lifestyle. Health Coaches engage patients and encourage them to take an active role in their health by providing them with the tools necessary to make healthy lifestyle choices and adopt life-long healthy behaviors. Minimum Qualifications: Education: Bachelor degree or higher in health promotion, Kinesiology, counseling, or related health field required. Coaching certification from a nationally recognized coaching organization is preferred. Experience: Health or Life coaching experience preferred. I: Essential Duties/Responsibilities (Position): The Health Coach designs, with the client, individualized action strategies and personal accountability structures that lead to greater control of the patient disease (e.g. diabetes, high cholesterol, asthma, etc.) and more positive clinical outcomes. Independently manages a caseload of clients and provides disease management and coaching services. Tracks and monitors the progress of the clients over time and ensures progress toward a measurable goal. Responsible for accurately recording pertinent patient data for the purposes of detailed record- keeping and tracking.
